Indianapolis, IN- Friday afternoon, July 20th, at approximately 2:30 p.m., Indiana State Police responded to a three vehicle crash on I-74 EB at the 99 mm.  (One half mile west of Acton Road)

Preliminary investigation revealed that Katrina Collins, 18, of Shelbyville, was driving a 1995 Pontiac EB I-74 at the 99 mm in the left lane and encountered a vehicle bumper in the lane of travel. The passenger car swerved to the right and side swiped an SUV. The passenger car then crossed the median and struck a WB semi tractor trailer and came to rest in the WB lanes.

The driver was ejected and pronounced dead at the scene. The front seat passenger, Melissa Collins, 20, sister of Katrina, was transported to Methodist Hospital by Lifeline in critical condition. A rear seat passenger, Brady Phares, 21, of Shelbyville, was transported to Methodist Hospital by ambulance and was also reported to be in critical condition.

Neither the driver of the semi-tractor trailer, Thomas Coppess, 47, of Anderson or the driver of the SUV, Kim Farlow, 57, were injured in the crash.

I-74 WB lanes were closed for approximately four hours while the crash was investigated and the wreckage cleared.
