Municipal License Plates
Below is a detailed list of required documentation needed in order to title and register municipal or law enforcement vehicles. Please ensure that the following information is included when mailing or delivering paperwork to the BMV in order to assist us in processing your request in a timely and efficient manner.
Title A Municipal Vehicle
- Certificate of Origin, Bill of Sale, or Statement of Manufacture
- Certificate of Gross Retail or Use Tax Exemption - (ST 108E)
- Odometer Statement

- For school buses, please provide a School Bus Inspection Form
- Application for Certificate of Title - State Form 44049
- Check or money order in the amount of $15
Register A Municipal Vehicle
- Affidavit of Eligibility for Municipally-Owned License Plates - State Form 53565
- Copy of title or Application for Certificate of Title - State Form 44049
- Color of the vehicle and Gross Vehicle Weight for school buses and trailers
- Insurance information

Transfer Plates Between Municipal Vehicles
- Copy of previous registration
- Affidavit of Eligibility for Municipally-Owned License Plates - State Form 53565
- Copy of the new vehicle's title or Application for Certificate of Title - State Form 44049
