You may now change your mailing address online by creating a personal myBMV account. Through myBMV you may register to receive e-mail reminders of your driver's license expiration date, print your driving record, and update your address and insurance information.
If you have an Indiana license, permit, or identification card you must notify the BMV of a change of address and apply for an amended license, permit, or identification card within 30 days of the address change.
A permanent address change must be completed at a BMV license branch so that you can be issued an amended license, permit, or identification card. You need to bring with you a computer-generated document such as a utility bill or pay check stub that includes your name and new address.

Before visiting a BMV license branch, you must visit a Social Security Administration office to officially change your name on Social Security documentation.
If your name has changed due to marriage you may take your certified United States Application of Marriage/Record or Marriage Certificate to any Indiana license branch to change your name on your license, permit, or identification card.
If your name has changed due to divorce you may take your certified divorce decree issued by a court to any Indiana license branch to change your name on your license, permit, or identification card.
If your name has changed for any reason other than marriage or divorce you may take your certified court decree issuing the change of name to any Indiana license branch to change your name on your license, permit, or identification card.
