To obtain a copy of your driver record you must submit Personal Information Request - State Form 48430.

Mail the completed form and the appropriate fee ($4.00 for basic driver record, $8.00 for a certified driver record) to:

Indiana Bureau of Motor Vehicles,
Attn: Driver Services,
100 North Senate Avenue, Room N-405,
Indianapolis, IN 46204

You may obtain a seven-year or a ten-year driver record. Please allow 7 to 10 business days for delivery. You may also obtain a driver record at any Indiana Bureau of Motor Vehicles reinstatement center.
