To renew or amend a license or ID card, the applicant must present:
Indiana applicants who are renewing or amending an Indiana driver license, permit, or ID card must surrender the license, permit, or ID card and verbally verify their Social Security number.
If the license, permit or ID Card has the correct address, no other proof of residency is required. If the applicant's address has changed, a document proving Indiana Residency must be presented. If the applicant's name has changed, a document proving name change--such as a marriage certificate or a court order--must be presented.
