Replace a Lost or Stolen License or ID Card
To replace a lost or stolen driver license, ID card, or learner/driver education permit, the applicant must present:
- One Primary Document
- One Secondary Document
- Proof of Social Security number or ineligibility
- Proof of Indiana residency, IF the applicant's address has changed since the driver license, ID card, or permit was issued or renewed.
A Primary or Secondary Document may also meet the Indiana residency requirement as long as the applicant's name and correct address are shown on the document. For a full list of acceptable primary, secondary, Social Security and residency documents, please download the
