Subscribe for e-mail updates

Print and complete the form you need, then return it to the address noted on the form. In order to ensure timely processing, be sure to fill out the form completely and to include any payment or additional documentation necessary for processing. You may not need to submit any of the following forms to get a license, permit, or identification card.

The Agreement of Financial Liability - State Form 39531 must be notarized and accompanied by a copy of the driver's license or identification card of the individual who is accepting financial responsibility for the minor.

Your Virtual License Branch

Skip your next license branch visit and create a myBMV account. A personal myBMV account is a virtual license branch that allows you to securely access many services online. Through your myBMV account you can print your driving record and register for e-mail reminders of your driver's license and license plate expiration dates.
