Division forms are available in 2 formats. Adobe Acrobat® .pdf files can be viewed and printed. To view .pdf forms you must use a copy ofthe Adobe Acrobat Reader.You may download the free Acrobat reader by clicking on button below.
The forms are also available for use as Microsoft Word 97® documents. The Word files can be completed on the screen prior to printing so that the user can print out a completed form.
To complete a Word form use the following procedure:
Note: If you use the Word forms frequently you may save them to your PC's hard drive to simplify the forms' completion process.
To save a form to your PC use the following procedure:
Important: To use the procedure noted above you must have access to MS Word 97®.
Problem: When I print the form it separates sections of the form and shifts them to the next page?
Solution: This is most often caused by the type of printer or print driver used to print the forms. Lower the margins established for the document in your Page setup and make sure you have the latest print driver installed. NOTE: Not all printers are capable of printing close to the borders of a page. If your printer will not print with margins of 0.25 inches you may not be able to correctly print the forms using Word. However, you may be able to print the forms from the .pdf format version.
Problem: When I attempt to print the file as an MS Word® document the data prints but the form boxes do not?
Solution: This problem can be corrected by using the following procedure:
Problem: When I click on a form I get an error message that says the form cannot be loaded or located?
Solution: Contact the site administrator
Problem: One or more of the fields in the form are not large enough to accomodate the information I need to enter.

Problem: I do not have MS Word 97 loaded on my computer but I would like to use the forms as Word documents. Can I do this?
Solution: Microsoft Internet Explorer® v 5.0 provides support for Word documents as fill-in forms. This means you can fill in and print them from the Internet site. However, as noted above you cannot save them to your hard drive and open them as forms unless you have access to MS Word 97®.
