This checklist will help you obtain all the information you need to order a permit in Indiana.
You must have an Oversize/Overweight Transporting Account set up with Indiana to obtain permits. You can set up an account electronically, by mail, or by fax.
Online - Go to the OSW Homepage and select "New to Indiana? Apply for an OSW Account". You must know your Federal Identification Number (FEIN), or your Social Security Number if you are a sole proprietor and have no FEIN. You must also know your USDOT Number (if applicable)
Fax or Mail in - You will need to complete the application Form M-203. (If you are a permitting service you will need form M-201).You must know your Federal Identification Number. You must also know your USDOT Number (if applicable). You may obtain all your forms electronically from this site.
If you intend to apply for permits electronically through the internet you must have a user ID and password. Once you have established an Indiana account you can register a user. Go to the OSW Homepage and select “First Time Users - Register for OSW".
Once you have an Indiana account and a user ID and password, you are ready to order your permit. You have several options when ordering permits:
Internet - Go to the OSW Homepage and select “Login to OSW”.
Mail-in or walk-in your request, on Form M-233, to 5252 Decatur Blvd. Suite R, Indianapolis, IN 46241
Permitting Services - You must obtain your clients’ Federal Identification Number and USDOT number to order a permit for them.
You must carry the general provisions Form M-204 in your vehicle at all times when operating with an Indiana Oversize/Overweight Vehicle Permit.
