Unified Carrier Registration (UCR)

New Interstate Carriers (crossing the state lines), Brokers, Leasing Companies and Freight Forwarders

Renewal Requirements for Existing Interstate Carriers (crossing the state lines), Brokers, Leasing Companies, and Freight Forwarders

            Form BAS-2

US DOT Numbers

Intrastate (never leave Indiana) For-Hire Carriers (hauling someone else's property) or Private Property Carriers of Hazardous Materials:

  • Form BAS-1
  • Fee is $10.00 per vehicle
  • Insurance filing Form E

Renewal Requirements for the above:

  • Form BAS-1(or preprinted form BAS-1A) plus fees paid and proof of insurance

Intrastate (never leave Indiana) Private Carriers (hauling your own private property) hauling non-hazardous materials:

Renewal Requirements for the above:

  • There is no renewal process for the above carrier.

Registrants (someone who leases onto someone else's authority)
(Interstate or Intrastate)

  • Nothing for Intrastate (unless you have your own Intrastate Motor Fuel Tax account, then you would submit completed Form BAS-1).
  • Interstate requires completed Form BAS-1 if you are getting an IRP plate or IFTA license.

Renewal Requirements for the above:

  • There is no renewal process for the above carrier.

Indiana Operating Authority

Intrastate Operating Authority for For-Hire Carriers Transporting Passengers or Household Goods:

New applicants for Operating Authority who are transporting passengers or household goods for hire:

Each application requires a $100 fee; plus a one time $80 publication fee.

Before granting or denying, the following is needed for each type of authority.

For Emergency Temporary Authority - If approved, valid for 30 days:

  • Duplicate copies of completed Form 703 (ETA)
  • Fees paid
  • Statement from applicant in affidavit form
  • Statement from supporters in affidavit form
  • Completed Form E if only intrastate or not based in Indiana
  • Tariff, which shows rates
  • A financial Statement
  • Completed Form BAS-1and fees paid if required.
  • Corporation papers from Secretary of State (certificate of Existence) if incorporated.
  • Attorney authorization (if Inc, LLC, or LLP)

For Temporary Authority - If approved, valid for 180 days:

  • Duplicate copies of completed Form 703 (ETA)
  • Fees paid
  • Statement from applicant in affidavit form
  • Statement from supporters in affidavit form
  • Completed Form E if only intrastate or not based in Indiana
  • Tariff, which shows rates
  • A financial Statement
  • Completed Form BAS-1and fees paid if required
  • Corporation papers from Secretary of State (certificate of Existence) if incorporated
  • Attorney authorization (if Inc, LLC, or LLP)
  • Form MCS-TA and fees of $2.50 per vehicle (after TA is approved)

For Permanent Authority - Takes approximately 3 months for approval depending on protests

  • Duplicate copies of completed Form 700
  • Fees Paid
  • Financial statement
  • Completed Form BAS-1and fees paid if required
