Transition visits for people moving out of state developmental centers and LP ICFs/MR
The Bureau of Quality Improvement Services (BQIS) monitors the transitions of individuals moving from state developmental centers and LP ICFs/MR. The intent of the monitoring is to assure that the transition is going smoothly, the support plan for the individual is being implemented appropriately, and any difficulties the individual may be experiencing due to the transition are identified and addressed appropriately. This monitoring includes completion of a standardized checklist at a minimum 7 and 30 days after the individual moves. Providers, including case managers, are informed of these visits during the transition process and are encouraged to participate.
When concerns are found during the visits, the Bureau of Developmental Disabilities Services (BDDS) coordinator is responsible for working with the provider(s) to develop a plan of correction that assures the concerns are resolved in a timely and appropriate manner. This plan of correction includes a description of specific steps that must be completed within specific timeframes as well as identification of responsible parties.
I recently completed a review of the transition process and found that approximately 80% of the concerns identified during the transition visits are not resolved within the timeframe specified on the plans of correction, nor are the BDDS service coordinators contacted with information as to why the deadline is not being met.
Effective immediately, I am requiring that the transition teams provide me a list of providers who have not met their deadlines for corrective action. This list will be reviewed by myself and Alison Becker, Director of BDDS, to determine what actions need to be taken. These actions may include freezing admissions if the concerns found during the transition monitoring have the potential of seriously impacting health and safety.
I am also advising that all providers, including case managers, work more closely with the BQIS staff and BDDS staff to assure that concerns are resolved appropriately, including communicating any barriers that arise during completion of a plan of correction.
