To:
Prospective and Existing Waiver and State Contract Providers
From:

Re:
Provider Orientation – Application Process
In an attempt to make the application process more understandable for the applicant and manageable by the Bureau of Developmental Disabilities Services (BDDS), a new application process is being implemented. This process covers those services within the DD, Autism, and Support Services Waivers and those funded within the State Contracts. Some services are also available within the Bureau of Aging and In-Home Services (BAIHS).
Changes include the following:
The first Orientation Session is scheduled for June 30, 2004 in the Indiana Government Center South. Registration begins at 8:30 am. The session will be from 9:00 am to 4:00 pm. It will provide information regarding provider applications for the services that require a detailed written proposal(s). This session will include presentations by the Bureau of Developmental Disabilities Services, the Bureau of Aging and In-Home Services (BAIHS), the Bureau of Quality Improvement Services (BQIS), and Electronic Data Systems (EDS). Those prospective providers that attend the Orientation Session will be able to submit application proposals from July 1 – 31, 2004 only. Registration begins June 10th. To register, please call Pearl Faust at 317-233-4848.
Another Orientation Session presenting information regarding provider applications for the services that require detailed written proposals will be scheduled in December. Acceptance of applications will be in January, 2005. The Orientation Sessions addressing the BDDS services that only require submission of documentation will occur in September and March. Acceptance of applications will be in October and April, respectively. The exact dates will be published in a separate bulletin.
Existing providers are not required, but strongly encouraged, to attend the orientation sessions. Important information from BDDS, BAIHS, BQIS and EDS will be provided.
If the existing provider wants to add new services and does not attend the Orientation Sessions, the provider may request the application form and submit only during the acceptance of application schedule listed above. Call Pearl Faust, 317-233-4848. The caller’s name must be provided as well as company name, and the application will be sent to the current address in the database.
The application packet includes Part 1 through 5. The applicant must indicate the services it wants to provide and then complete and submit all relevant Parts of the application. After attending the Orientation Session addressing the detailed written proposal services, applicants can apply for both types of services (detailed written proposal services and those which only require the submission of documentation). New prospective providers may only apply for the detailed written proposal services after attending the session addressing that service type.
BDDS Services requiring detailed written proposals are:
BDDS Services requiring submission of documentation only:
NOTE: Case Management (waiver) and Targeted Case Management – Deinstitutional Services, the provider requirements and the application process are being reviewed currently and these will be addressed in a future bulletin.
Applicants that are interested in applying for the Medical Model Waivers services only may apply at any time to the Bureau of Aging and In-Home Services.
