Money Follows the Person - Psychiatric Rehabilitation Treatment Facility (MFP-PRTF) Transition Waiver

The Division of Aging is responsible for overseeing the daily management of the MFP program in regards to youth leaving Psychiatric Residential Treatment Facilities (PRTFs). The Division of Aging works in concert with other divisions to implement the program.

 The Division of Mental Health and Addiction has the following responsibilities:

  • Approving and enrolling all providers of MFP services to youth leaving PRTFs
  • Training and certification of Wraparound Facilitators

The Division of Aging has the following responsibilities:

  • Responsible for oversight of MFP program activity, level of care determinations, and service plan reviews
  • Reviewing and approving all manuals, bulletins, and communications
  • Final Authority for approvals
