Welcome to the certification and licensing section of the DMHA website. There are several certifications/licenses available. The following list provides a brief description of the available certifications/licenses as well as listing all forms and material needed to begin the process of becoming certified/licensed. All required attachment information and forms are available on this site.

ADDICTION SERVICES CERTIFICATION ( See 440 IAC 4.4):

All providers of addiction treatment services must be certified by DMHA. Veterans' Administration and University programs are exempted. If an agency does ONLY intervention, assessment, referral, education and self-help groups, such as AA, NA, etc. it does not need to be certified. There are two certifications:

1) Addiction Services Regular certification, for an agency of eleven or more direct service staff or any new opioid treatment program. The agency has 12 months in which to apply for accreditation by an accrediting body approved by DMHA and an additional 12 months in which to achieve that accreditation. Until accreditation is attained, the agency must have at least one direct service provider staff with a credential approved by DMHA.

2) Addiction Services Outpatient certification for agencies of one to ten direct service staff. It requires at least one direct service provider staff with a credential from a credentialing body approved by DMHA. If the agency has more than one direct service provider staff, the agency must additionally have at least 50% of the direct service provider staff with a credential or license approved by DMHA.

Each application requires the application form, the facility fact record for each site of service and the materials requested in the Required Attachments: Addiction Services 440 IAC 4.4. IN ADDITION, the Addiction Services Outpatient application requires the Documentation of Direct Service Providers form and the Statement of Understanding of and Compliance with the Amended Rule form.

RESIDENTIAL CARE PROVIDER CERTIFICATION (See 440 IAC 6):

Community Mental Health Centers, Managed Care Providers and Addiction Services Regular providers are deemed Residential Care Providers (RCP) by virtue of their DMHA certification and accreditation.

Other agencies may become a Residential Care Provider by making application to DMHA. All RCPs must have a governing board with at least one consumer member; a CEO or director with a Masters degree; among other requirements of 440 IAC 6. RCPs may operate Transitional Living facilities, Supervised Group Living (SGL) facilities or Sub Acute Living facilities as long as they meet the requirements of 440 IAC 7.5. The SGL and Sub Acute programs must have a separate license from DMHA to operate.

Each application requires the application form, the facility fact record for each site of service and the materials requested in the Required Attachments for Residential Care Provider Certification.

SUPERVISED GROUP LIVING and SUB ACUTE LICENSES (See 440 IAC 7.5):

Each of these programs must have a license from DMHA to operate. An applicant must be a Community Mental Health Center, a Managed Care Provider, an Addiction Services Regular provider or a Residential Care Provider.

Each application requires the application form, the facility fact record for each site of service and the materials requested in the Required Attachments for either Supervised Group Living Facility or Sub Acute Facility (for whichever is being applied).

COMMUNITY MENTAL HEALTH CENTER CERTIFICATION (See 440 IAC 4.1):

 No new applications are being accepted at this time.

MANAGED CARE PROVIDER CERTIFICATION 440 IAC 4.3:

Applications for SFY11 will be accepted in the fall of 2009.  DMHA will be accepting applications only from current Managed Care Providers (MCPs) and non-profit Panel Members of MCP networks certified as of SFY09. 

MCP contracts have a minimum dollar amount of $350,000. Therefore, applicants must be currently receiving DMHA allocated funds of at least $350,000 in Hoosier Assurance Plan services. 

Except for the Compulsive Gambling Addiction population, applications from MCPs and MCP Panel Members will be accepted only for the populations which the MCP or MCP Panel Member is approved to serve in SFY10.  Applications to serve the Compulsive Gambling Addiction population in SFY11 will be accepted from all MCPs and MCP Panel Members.

PRIVATE MENTAL HEALTH INSTITUTION (HOSPITAL) CERTIFICATION (See 440 IAC 1.5) :
