If you know someone who would benefit from the services of Indiana Access to Recovery:

  • Find out if the individual resides in one of the seven ATR counties (Allen, Elkhart, Lake, Marion, St. Joseph, Vanderburgh or Vigo)
  • If so, select/print the ATR Client Choice Form for that county
  • Review and complete the form with the potential client
  • Fax the completed form to the Recovery Consultation agency selected by the client
  • Call the Recovery Consultation agency to let them know you have sent a referral
  • DOC Referrals: Additionally, please fax a copy of the Client Choice Form to Amanda Copeland at 317-233-1474

For more information about each Recovery Consultation agency, please visit the “Provider Locator” link. Indiana Access to Recovery continually recruits new providers. Please ensure that you have the most up-to-date form by checking the website every few weeks for the latest Client Choice Form (the last revision date is located on the upper right corner of the form.)
