Instructions and forms used to make a paper submission of the Emissions Statement. These are not necessary if an electronic submission is made.

Hardcopy Instructions

Blank forms can be found in the forms catalog on the IDEM Forms page in either Microsoft Word or Adobe Acrobat formats. If you have any questions regarding these forms please contact state staff. The required forms are:

  • AES-01 Air Emission Statement Certification State Form - 52052
  • AES-02 General Facility Information State Form - 52053
  • AES-03 Stack Information State Form - 52054
  • AES-04 Abatement Equipment State Form - 52055
  • AES-05 Group/Process/Emissions Information State Form - 52056
  • AES-06 Voluntary Hazardous Air Pollutant Reporting Form - 52057
