NOTE: The application process for the Wellness Tax Credit has changed, as of 8.20.09.
Please complete this application form as thoroughly as possible, including descriptions and examples from your company’s wellness plan. For guidance, reference the Certification Criteria and examples of other approved plans. Your plan should be no more than 5 pages in length.
The application must be submitted every year for advisory board review, even if your plan was approved the previous year. Once submitted, the board will meet and approve or defer your plan. Notification of your application status will be sent shortly after the advisory board meets.
To receive certification in 2009, all plans must be submitted by November 15th, 2009.
