OUTREACH TESTING: Outreach Testing funding calls for an individual level intervention which will ensure that that persons at increased risk for HIV and HCV are properly screened and tested. Furthermore, those found to be infected shall be immediately linked into the existing system of care.
LINKAGE TO CARE: The grantee will provide Linkage to Care services focusing on high-risk negatives, HIV-positive individuals who have fallen out of care or who may be at risk for falling out of care.
PrEP/ART: Will provide PrEP services to individuals who meet the criteria for such and will educate PrEP recipients on the importance of treatment adherence.
CLEAR: CLEAR is intensive, individualized, client-centered counseling for adopting and maintaining HIV risk-reduction behaviors for HIV positive individuals who continue high-risk behaviors, discordant couples, and HIV negative individuals that engage in high-risk behaviors.
SPECIAL POPULATIONS SUPPORT PROGRAM (SPSP): The program is designed to cover two complementary services: disease prevention and supportive care. The Special Populations Support Program strives to minimize the negative impact of chemical dependency on the health of persons at risk of HIV, or those already living with HIV, in a compassionate and non‑judgmental environment, cost-free to the client
