Meeting began at 12:37 p.m. The mission statement was read by Bryan Amador and quorum was established.
April Minutes – Bryan motioned and Carrie seconded. April minutes were approved as written.
Elections for Vice Chair were held via e-mail and there was a 2 way tie between Noelle Redmond and Jeff Wolfe. The final vote came from Bryan Amador and Jeff Wolfe will be the new Vice Chair of CHSPAC.
Planning Committee – All tasks have been completed on what can be modified. There is a huge percentage of consumers that are needing dental due to there being little or none available. There is a potential to apply through Part F and the dental school. We have set up a Dental AD HOC committee consisting of April Grudi, Kellie Kaneshiro, Tammy Morris, Ricky Conrad, and Jeff Wolfe.
Medical AD HOC – Consumers Jody Grieb, Mia Gregory, and Sharon Erdiman have been added to the Medical AD HOC Committee.
Comprehensive Plan – We got the major narrative finished but the Resource Guide is not yet finished being updated and the Unmet needs report is outstanding.
Consumer Forum-Clarksville – There was a “Learning and Living Well” Consumer Forum in Clarksville Indiana on May 28, 2009. Fourth Quarter surveys were gone over and there was 22 in attendance. We also held a Consumer Forum in Merrillville on April 17, 2009 and there were 33 in attendance.
Meet & Greets – Jeff explained to the rest of the CHSPAC board how CAB is handling the scheduling and the locations of the meet and greets.
Dr. Carrie Foote, Ph.D. did a presentation on the Results: Indiana Consumer Survey.
Care Coordination Presentation – Spotlight had recognized a few of the Care Coordinators and Kristi wanted to recognize them publicly.
We would like to thank all of the Care Coordinators for their time and diligence in serving our clients.
Jeff announced that we have 5 new CAB Representatives.
CAB voted to integrate to a subcommittee of CHSPAC and there will be more information forthcoming.
Tracy is selling “Beads of Life” jewelry made by India ladies from Uganda.
