How do I apply for WIC?
Eligibility Requirements
First, please read the eligibility requirements for WIC assistance. Then, if you think you qualify:
Call the WIC clinic location nearest to your residence. On the WIC Web page clinic location map, click on your county of residence, and you will see the contact information. If you prefer to contact the state office to find the clinic nearest you, please call toll-free 1-800-522-0874, or email inwic@isdh.in.gov. When you call your local clinic, the staff will be able to answer your questions and get you started with a certification appointment.
