The Indiana State Department of Health (ISDH) is pleased to present the ISDH Healthcare Associated Infection (HAI) Consumer Brochure. The brochure was created as part of the ISDH Healthcare Associated Infection Initiative and first published in April 2011. A Spanish version of the brochure was developed and released in January 2012. The brochure describes healthcare associated infections, how infections are transmitted, and how residents, patients, and families can be partners in preventing infections.
The ISDH Pressure Ulcer Consumer Brochure is available to Indiana health care facilities, organizations, and individuals at no cost while supplies last. The brochures come in packets of 50 brochures or single copies. To order brochures, contact Nancy Gilbert of the Indiana State Department of Health at (317) 233-7616 or ngilbert@isdh.in.gov.
The brochure was designed to assist health care facilities, clinics, and agencies in their resident and patient education and communication efforts. Facilities are recommended to provide the brochure as part of each care planning conference and use it as an opportunity to discuss infection prevention.
The Healthcare Associated Infection Consumer Brochure was developed as part of the ISDH Healthcare Associated Infection Initiative. The development and printing of the brochure was funded by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS) Civil Money Penalty Fund. The Indiana Initiative ran from September 2, 2009 through December 31, 2011.
The Initiative Collaborative Team participated in the development of the brochure. The brochure was designed by Rick Field, PhD, Cabello Associates, in coordination with the ISDH and University of Indianapolis Center for Aging & Community. Contributing to the content for the brochure included:
