The Charter School Loan Forgiveness Application must be completed and submitted to OMB as specified by June 7, 2013. Please follow all of the directions below to ensure your application is completed on time.
Directions to apply:
1.) Download the application and instructions here:
2.) Once you have completed the application, please save and email a digital copy of it (in a Word format) to the following email address: CharterLoans@omb.in.gov. When sending your email, please use the following format for the email subject and file name:
'Charter Loan Application - Your Charter School Number'
3.) Once you have emailed the application, you must print off and mail a hard copy of the application with an original signature to the following address:

Upon receipt of both the electronic and hard copy application materials, the applicant will receive confirmation of receipt and forgiveness no later than June 21, 2013.
