To file a complaint with the OUCC's consumer services staff, please fill out this form as completely as possible. Your full name, street address and telephone number are required in order for us to respond to your complaint.

Note: The OUCC will not voluntarily share your information (including your name) publicly, unless you wish otherwise. However, the agency may be compelled by a court or regulatory body to produce copies of complaints under the Indiana Access to Public Records Act (IC 5-14-3-1 et.seq.).

Once you have submitted your complaint, you should be directed to a screen showing that your complaint has been successfully submitted. After your complaint is submitted and you have reviewed the confirmation screen, simply click the "back" button on your browser to continue using the OUCC Website. An OUCC consumer services representative will review your complaint and contact you in the near future.
