Renewal Dates/Information
Physical Therapy licenses and Physical Therapy Assistant certifications expire on June 30 of even-numbered years. The next renewal date is JUNE 30, 2014.
ONLINE RENEWAL - A license renewal email notice is emailed to the licensee's email address of record with the Professional Licensing Agency sixty (60) days prior to the expiration of the license. Information is provided in the email on how to renew your license online. If you do not receive your renewal email notice, please contact our office by calling (317) 234-2051 or by email at pla6@pla.in.gov. Please be sure to give us your name, current address, license number, profession, email address and telephone number.
Address Change - You are required to notify the Professional Licensing Agency of any address changes. You can update your address during the renewal process or anytime using the License Express option. You can also notify our office in writing. Your written notification must include your name, license number, profession, social security number, date of birth, and both your old and new addresses. Please send the information to the Professional Licensing Agency at 402 West Washington Street, Room W072, Indianapolis, Indiana 46204, e-mail at pla6@pla.IN.gov, or fax it to (317) 233-4236.
Name Change - You may have the name changed on your licensure records by submitting a copy of an official name change document such as a marriage certificate or a divorce decree. A copy of a social security card will NOT be accepted. Please send the information to the Professional Licensing Agency at 402 West Washington Street, Room W072, Indianapolis, Indiana 46204, or fax the information to (317) 233-4236. You also need to include your name as it is listed now, your new name, your license number, profession, social security number, and date of birth. If you wish to order a new pocketcard or wall certificate due to the name change, you can do so using the License Express option.
