HOW TO RENEW
OPTION #1 – RENEW ONLINE!!!
The Professional Licensing Agency is proud to offer one of its newest services, My Indiana Health License, which allows you to renew your licenses online using a Visa or MasterCard credit card or debit card on our secure server. If you would like to renew online, you may do so by returning to the Professional Licensing Agency homepage and clicking on License Express.
OPTION #2 – RENEW VIA PAPER RENEWAL
If you would prefer to renew using our paper renewal applications, you will need to contact our office at (317) 63RENEW or email us at pla5@pla.in.gov. Please note that renewing by mail may take up to 10 business days to process after the Professional Licensing Agency receives your completed renewal form.
INSTRUCTIONS FOR PAPER RENEWAL APPLICATIONS
Make any address or name corrections on the renewal application and submit/attach supporting documentation (i.e. Copy of marriage certificate or divorce decree) showing legal name change.
Answer all the questions (1–5) on the renewal application. If you answer "YES" to any question(s) on the renewal application, please attach a notarized affidavit with sufficient information to explain the positive response(s).
Sign and date your renewal application.
Enclose a check or money order for the $100.00 renewal fee, made payable to the Professional Licensing Agency.
PLEASE BE ADVISED THAT IF ALL THE ABOVE LISTED INFORMATION IS NOT COMPLETED, YOUR RENEWAL AND CHECK WILL BE RETURNED TO YOU FOR COMPLETION AND YOUR RENEWAL WILL BE DELAYED.
LATE RENEWALS -- PENALTY FEES
If a renewal application is postmarked after June 30, 2013 the license automatically becomes invalid. The license may be renewed within two (2) years of the date of expiration by paying a late fee of $50.00 in the addition to the normal renewal fee of $100.00 and completion of the continuing education requirement.
The renewal application must be postmarked by June 30, 2013 to avoid the payment of any late fee.
Inactive Status: To request inactive status, please return your completed renewal application, a $50.00 renewal fee and a statement notifying the Board that (1) you do not maintain an office for the practice of podiatric medicine and (2) that you do not charge for any podiatric medical services that you might render. To reactivate your inactive license you must submit a completed renewal application, a renewal fee of $100.00 and meet your continuing education requirements.
DUPLICATE POCKETCARDS - In order to receive a duplicate pocketcard, you will need to go to our Homepage and click on License Express.
