VERIFICATION LINE & FREE ONLINE VERIFICATION 
The Indiana Professional Licensing Agency will verify the status of a license or registration by calling (317) 234-2054 or you may obtain basic information on a practitioner's license or registration for free by accessing our free online verification search.

VERIFICATION OF LICENSURE TO ANOTHER STATE 
This is a verification of an Indiana license to another state.  The state of Indiana is now providing a Digital Certification of the license that you can download and email to your state board.   This is an official statement of the basis for licensure and includes the practitioner's name, profession, license number, status and any other conditions/restrictions of the license.  Click the link below to order your digital certification.

LICENSE CARDS/WALL CERTIFICATES 
Initial and additional license cards may be obtained at Services.IN.gov/License Express.   You may purchase a blue license card to be mailed to you or download a free license card for immediate printing.   This service is available 24 hours a day, 7 days a week.

Complaints against a practitioner are investigated by the Office of the Attorney General - Consumer Protection Division.
