BARBER APPLICATIONS AND FORMS
Barber Examination Application Instructions
- File completed application, including the portion your school must complete.
- Complete 1500 hours of instruction in a barber school.
- Include a 3" X 5" photo of yourself.
- Include the application fee of $40. If you have a conviction record, include a copy of the conviction record that indicates the disposition of the conviction. It should include the type and date of the conviction, time served, if applicable, and the disposition of any probation being served or successfully completed.
- If you are requesting a temporary permit, include the temporary permit request and the $10.00 temporary permit fee.
Barber License by Reciprocity Application Requirements
- File completed application.
- Include a 3" X 5" photo of yourself.
- If you have a conviction record, include copies of the court records explaining the conviction and a letter of good character from an authority figure such as a barber instructor, civic leader, or pastor.
- Include a license certification from the state or country you hold a license in. (if from a foreign country, must include an English translation of the documents)
- The state or country you hold a license in must require at least 1500 hours of education or one (1) year of licensed practice is equal to one hundred (100) hours of education to an applicant who has completed a minimum of one thousand (1000) hours of education. The jurisdiction must also require at least 75% passing on the written and practical exams.
- Include the license fee (of $40).
- If you have a conviction record, include a copy of the conviction record that indicates the disposition of the conviction. It should include the type and date of the conviction, time served, if applicable, and the disposition of any probation being served or successfully completed.

- Complete 900 hours of instruction in a barber school
- Include a 3" X 5" photo of yourself.
- Include the application fee of $40.
- If you have a conviction record, include a copy of the conviction record that indicates the disposition of the conviction. It should include the type and date of the conviction, time served, if applicable, and the disposition of any probation being served or successfully completed.
- Must hold an active Indiana barber license.

Instructions
- File the completed application.
- Must have a licensed barber to supervise the shop.
- Include the license fee of $40.
- Include the $10.00 temporary permit fee. A temporary permit will not be issued without this fee.
MOBILE SALON (BARBER SHOP)
• A mobile salon/shop may be;
(1) a self-contained facility that may be moved, towed, or transported from one location to another and in which cosmetology, electrology, esthetics, manicuring or barbering is practiced, or;
(2) A business in which cosmetology, electrology, esthetics, manicuring, or barbering equipment is transported to and used on a temporary basis at a location other than a selected salon site, including:
(A) other cosmetology, electrology, esthetic, manicuring, or barbering salons/shops;
(B) clients' homes; and
(C) nursing homes.
• Applicant comply with the rules adopted by the board (820 IAC 2-1, 3-1, and 3-2)
• Mobile salon must be under the personal supervision of a person who has at least six (6) months active experience as a beauty culture professional before the application is submitted.
Application for Mobile Salon License
