Installer Application

Please include the following items when applying:

  • Completed and notarized application
  • Application fee - $150
  • Proof of insurance or surety bond
  • Certificate of completion for board approved training course
  • Recent photograph-Passport quality
  • Notarized professional reference form OR Notarized verification of supervised experience form
  • The application fee of $150.00 should be made payable to Indiana Professional Licensing Agency (IPLA) and must accompany the application.

    All applicants must be able to show that she/he has at least one (1) year of work experience as an installer of manufactured homes. IMPORTANT NOTE: The experience must be explained in detail, listing all duties performed and how they were accomplished.

    Only one of the following should be submitted.

    1. The Verification of Supervised Experience form must be completed by a current or former employer. This person must hold an active manufactured home installer's license. You must be able to show at least one (1) year of experience as a manufactured home installer. The applicant cannot verify his/her own experience.
    2. The Professional Reference form must be completed and submitted by three (3) references, which are familiar with the applicant's work experience and competency. Two (2) of these references must be licensed manufactured home installers.

    Proof of general liability insurance with products/completed operations coverage for the minimum coverage of $100,000 per occurrence and $1,000,000 aggregate OR a surety bond in the amount of $100,000.


    Application for approval of Installer Pre-Licensing Course Provider or Continued Education Course Sponsor

    Please include the following items when applying:

  • Completed and notarized application
  • Descriptive course outline for each course
  • Professional biography for each instructor
  • Sample course completion certificate
  • Course evaluation form
  • Instructors must posses special skill or knowledge of the subject being presented and have at least one (1) of the following minimum qualifications:

    1. An instructor of manufactured home installation teaching at an accredited institution of higher education in the United States or comparable school of a foreign country
    2. Have a college degree related to the material that the person is to teach
    3. Five (5) years full-time experience in a profession, trade, or technical occupation related to the material being taught
