Registered nurse – expires 10/31/odd numbered years
Nurse midwife – expires 10/31/odd numbered years
Advanced practice nurse prescriptive authority – expires 10/31/odd numbered years

If your license has been expired more than 3 years you will need to submit a renewal form, $100.00 check or money order (Made out to IPLA) and a copy of a current license. If you do not have a current license, please follow these instructions:
As of September 15, 2016, RN and LPN licensees wishing to activate a license expired more than 3 years must fulfill the continuing education requirement as well as complete a refresher program with a clinical component. The continuing education requirement is satisfied upon providing proof of completing 24 contact hours of continuing education for nurses from an accredited provider of CE for nurses: 6 contact hours each, dealing with the following 4 subject areas:
CEs can be found at www.RN.com or a comparable website. Please send in a copy of your CEs with your renewal form. Once a renewal form is submitted, you have 30 days to submit the CEs documentation. If you go past the 30 days, you will be required to make an appearance before the board.
