DEALER/DEALER BRANCH/TRANSFER/WHOLESALE

  • Application **
  • Registered Retail Merchant Certificate **
  • Zoning completed on bottom of application by local zoning board
  • Insurance (Certificate of Liability or SR23) **
  • Pictures of building, lot, sign and hours of operation
  • Franchise agreement if new is marked on the application
  • Corporation or Organization registered with Indiana Secretary of State’s Business Services
  • Please provide Federal ID number with documentation from Internal Revenue Service
  • Please provide Social Security Numbers for owners/partners/directors/officers listed on application

MANUFACTURER / CONVERTER MANUFACTURER

  • Application **
  • Registered Retail Merchant Certificate **
  • Zoning completed on bottom of application by local zoning board ( if located in Indiana).
  • Insurance (Certificate of Liability or SR23) **
  • Pictures of building, lot, sign and hours of operation
  • If Corporation, LLC, or Organization document showing that business is registered with Indiana Secretary of State’s Business Services
  • Please provide Federal ID number with documentation from Internal Revenue Service
  • Please provide Social Security Numbers for owners/partners/directors/officers listed on application.

SALVAGE

  • Application **
  • Registered Retail Merchant Certificate **
  • Zoning completed on bottom of application by local zoning board
  • If Corporation, LLC, or Organization document showing that business is registered with Indiana Secretary of State’s Business Services
  • Please provide Federal ID number with documentation from Internal Revenue Service
  • Please provide Social Security Numbers for owners/partners/directors/officers listed on application
