Tobacco incentive back for second year

That $500 that was used this year to offset your health insurance deductible? Good news - it's back for 2009! And like last year, all you need to do is not use tobacco products. Avoid them and you'll have $500 toward your health insurance deductible.

The tobacco-free incentive is available to all state employees who agree to forego the use of tobacco.

Please note, if you find that at any time during the year you can not refrain from tobacco use, the state asks you to contact the State Personnel Benefits Division to have your deductible reinstated. Remember, all state employees may be subject to testing for nicotine.

Contact SPD Benefits by phone at (317) 232-1167 (local Indianapolis) or (877) 248-0007 (outside Indianapolis. You can also e-mail SPD Benefits at spdbenefits@spd.in.gov.

The Agreement You Will Sign

In exchange for a $500 reduction in my state employee group health insurance deductible:

  1. I agree to abstain from the use of any tobacco products during 2009.
  2. I understand that in order to receive the reduction in the deductible, I may be subject to testing for nicotine, and I agree to submit to such testing.
  3. I understand that if I fail to honor this agreement, the full insurance deductible will apply to my 2009 health care expenses. This consequence will occur whether my failure is self-identified or determined by the State.
  4. I understand that dishonesty may result in disciplinary action up to and including termination.
