Delta Dental does not send ID cards so you will need to print one by following the instructions below. Your provider will need to see your ID card when you receive services.

Go to www.deltadentalin.com and click on the triangle next to Select a Tool Kit. On the upper left side of the screen, click on Consumer Tool Kit which will take you to a log in page. You will need to register to access your personal information and to log in. On the left side of the next screen, click on the Print ID Card link. Individual ID cards are not available for dependents.

After you have printed your card, be sure to log out.
