The Request for Hardship Waiver - State Form 28622 is only intended to waive age, experience, and/or practice driving requirements for the initial issuance of a driver's license.
This form cannot be used if the applicant’s driving privileges are currently or have been suspended, revoked, invalidated, cancelled, or subject to similar action or if the applicant is attempting to secure hardship, probationary, conditional or specialized driving privileges.
If the applicant has previously held an unsuspended learner’s permit for 180 days or more, then the applicant does not need to apply for a hardship waiver.
