A Statement of Intent to Terminate as a Senior Judge form is provided to senior judges wishing to end their service as a Senior Judge. This form should be completed and submitted through the Indiana Courts Portal to notify the Office of Judicial Administration of the end of their service as senior judge. When possible, statements of intent should be submitted at least two weeks prior to the end of intended senior judge service. In lieu of the form, a letter in PDF format may be submitted.
Instructions for submitting a statement of intent to terminate

Use the free Adobe Reader or any professional version of Adobe Acrobat to ensure your entries are saved. Do not print and handwrite your entries and do not scan to PDF. If a letter is preferred, please save the file as a PDF before uploading.

Sign into the Indiana Courts Portal at portal.courts.in.gov. If you are not sure how to sign in, see the help topic on signing in or creating an account.
- From the dashboard in the "My tools & settings" section, click on "Upload a document"
- From the menu, choose the document type: "Senior Judge Statement of Intent to Terminate"
- Click the browse button to locate your saved PDF and click "Submit"
- After submitting, you'll see a list of all documents you've uploaded successfully. Documents marked with a "Received" status can be withdrawn. Documents that are marked with a "Submitted" status cannot be withdrawn. You may withdraw your document for approximately 60 minutes after uploading before it is considered submitted.
