Administration for Community Living has created 2 informational cards that capture important information that emergency responders and medical personnel can use during an emergency situation; they can be found at the below links, completed, and kept in a readily accessible location: Medical Emergency Card and Communication Assistance Card

State Health Insurance Assistance Information Program

A free counseling program provided by the Administration on Community Living and the Indiana Department of Insurance for people with Medicare.  For free unbiased health insurance information contact a counselor at 800-452-4800 or visit https://www.in.gov/ship/ for more information.

Department of Natural Resources- Golden Hoosier Passport

A permit issued to Indiana residents who are at least 65 years and older. Allows admission to any property administered by the Indiana Department of Natural Resources in which fees are charged (not valid for entrance to Indiana State Museum, State Historic Sites, or Falls of the Ohio Interpretive Center). FSSA INconnect App

The INconnect app allows you to search for FSSA services and Medicaid providers in Indiana and to take an assessment of your needs.

Medicare.gov Pharmaceutical Assistance Program

A pharmaceutical assistance locator that helps individuals locate companies that have programs to help pay for medication for people enrolled in Medicate prescription drug coverage (Part D). Click here for the assistance locator.

National Council on Aging Benefits CheckUp

Https://www.benefitscheckup.org/ is an all- inclusive website with access to over 2500 benefit programs nationwide. Involves completing a questionnaire online which enables an individual to find out what benefits may be available in their particular area.
