Impacts to any state regulated wetland require that you submit the Application for Authorization to Discharge Dredged or Fill Material to Isolated Wetlands and/or Waters of the State application. You should also include your wetland classification worksheets for all wetlands in the project area if you have not already received a Waters of the State Determination (WOSD) or a copy of the Waters of the State Determination.

Where do I get applications from the U.S. Army Corps of Engineers or the Indiana Department of Natural Resources?
There are three (3) districts of the U.S. Army Corps of Engineers located in Indiana. The Detroit District of the U.S. Army Corps of Engineers covers the northern portions of the state. The Chicago District of the U.S. Army Corps of Engineers covers a small portion of northwest Indiana primarily associated with the drainage area of Lake Michigan. The Louisville District of the U.S. Army Corps of Engineers covers the majority of the state except for northern counties covered by Chicago and Detroit.
