IC 27-1-22-8
Rating organizations must hold a letter of registration to do business in Indiana. All licenses must be renewed every three (3) years. To complete the renewal, please submit the following and tab the items according to the numbers below.
1. Application
2. Filing Fee of $75 to be remit via NAIC OPTins system
3. A list of members
In lieu of #3 above, the organization may submit A. below:
A. Non-Indiana organization may submit the report of examination made by the insurance supervisory official of another state for compliance with IC 27-1-22-15. This report may not be older than three (3) years.
The below items are only required if changes have been made since the last renewal:
B. A copy of the organizations constitution, articles of agreement or association or its certificate of incorporation
C. A copy of the organizations bylaws, rules, and regulations governing the conduct of its business.
D. The name and address of a resident of this state upon whom notices or orders of the commissioner or processing affecting such organization may be served. (Uniform Consent for Service of Process, Form 12)

After the renewal is approved, a new Indiana Certificate of Authority will be mailed.
