The Health Department provides immunizations at NO COST for children who are on Medicaid, who are uninsured, underinsured, or an American Indian/Alaskan native. We can also bill most insurance companies for childhood immunizations. We administer vaccines for children (birth through 18 years) by appointment, Monday and Wednesday, 9:30 AM to 5:00 PM and Tuesday and Thursday, 8:30 AM to 4:00 PM. Call 812-738-3237, Option 1, to make an appointment. Office hours are Monday and Wednesday, 9:00 AM to 5:30 PM, and Tuesday, Thursday and Friday, 8:00 AM to 4:30 PM.
Any shot received at the health department is recorded in the Children & Hoosiers Immunization Registry Program (CHIRP). Doctor's offices have access to CHIRP, and so do all Indiana county health departments. Any immunization received at any Indiana county health department is on the CHIRP database, which is accessible to qualified personnel from the internet at any time.
NOTE: To bring a child for immunizations, you must be their parent or legal guardian. If you are not (if you are a step-parent, grandparent, babysitter, or another relative), you MUST bring a note signed by the parent or guardian stating that it is okay for you to bring the child for their immunizations.
Also, bring an updated copy of the child's shot record and an insurance card for those who are insured.
