Birth and Death Records
Certified copies of Birth or Death Certificates can be obtained by visiting our office or can be ordered by either mail or internet/fax.
Fees for Records:
- Certified Birth Certificates: $15.00 (per certificate)
- Affidavit Requesting Amendment: $10.00
- Certified Death Certificate: $20.00 (per certificate)
- Un-Certified Birth/Death Certificate: $10.00 (per certificate)
- Paternity Affidavit: $40.00 (each)
- Paternity Affidavit Upon Marriage: $40.00
- Genealogy Search Per Name: $5.00
- Genealogy Hard Copy: $10.00
WE ACCEPT CASH, CREDIT CARD, LOCAL CHECK, OR MONEY ORDER.
Birth and Death Certificates
Reasons that a Certified (raised seal) Birth/Death Record could be required:
- International Travel (passport application)
- Insurance Purposes
- School Registration
- Sport Registration
- Sale of Vehicle, Home, Property, etc.
- Retirement
- SSI
Guidelines for Requesting Certified (raised seal) Birth or Death Records (We only have records for people who were born or died within Huntington County!):
- Completed Application (available online)
- Self-Addressed Stamped Envelope
- Birth/Death recording start date is 1882
- Photo ID required
*For birth and death records occurring in other Indiana counties, please contact that county health department regarding application procedures and fees. A list of Indiana county health departments and their websites can be found at www.in.gov/isdh/24822.htm. If you were born in another state, or wish to obtain a family member's death certificate, you can apply online at www.vitalchek.com, or contact the Health Department in the state in which the event occurred.
