Controlled Substance Registration

A controlled substance registration (CSR) must be obtained directly through the Indiana Professional Licensing Agency (PLA), which issues that specific license in partnership with the Pharmacy Board.

CSR-VETERINARIANS ONLY: IC 35-48-3-3 has been amended to provide that a person licensed to practice veterinary medicine in Indiana holding an Indiana controlled substance registration is not required to obtain a separate registration to possess or dispense controlled substances in the usual course of veterinary practice at a site, so long as the site is not a principal place of business or professional practice.

CSR RENEWALS: Submit the renewal after renewing your veterinary license.

Obtain a CSR

To obtain a CSR, use the MyLicense.in.gov portal to apply. Veterinarians must have an active license to obtain a CSR. Individuals applying for an initial license may simultaneously apply for a CSR; however, the veterinary license must be processed before the CSR can be issued.

CSR INFORMATION UPDATES/CHANGES: To make updates, such as email or postal address, you DO NOT need to obtain a new CSR. Renew the CSR (after license renewal), then email information updates to the Professional Licensing Agency at:  pla5@pla.in.gov.

To maintain a CSR, Indiana requires veterinarians to complete 2 hours of continuing education related to opioids. See the PLA link below for guidance on approved sources of opioid education.
