Office of Early Childhood and Out-of-School Learning Complaint Process
The OECOSL has no authority over areas such as operational policies about child care or staff payment, wages, days/hours of operation or general business practices. Any person who suspects that child abuse or neglect may have occurred in a child care program has a duty to report to the Department of Child Services at 800-800-5556.
Examples of issues that might be reported include:
- Children not being fed nutritious meals.
- Activities or materials not appropriate for the ages of children in care
- Staff responsible for too many children
- Children being left in classrooms without an adult present.
The division offers several ways to file a complaint:
- Call 877-511-1144 (in-state only) or ask to speak to someone to the licensing unit.
- Fax information to the office at 317-234-6905.

When filing a complaint, please be prepared to share the following information:
- The name of the child care program.
- The true address of the child care program.
- Details about your concern.
- Any information about individuals involved in the incident.
- Approximate date, time, and the location where the incident occurred.
If you would like our office staff to follow up with you after the completion and review of the complaint, you will need to provide your name along with a valid email address and phone number.
All complaints will be entered into our Child Care Information System within 48 hours of being received.
All complaints will be reviewed and an inspection or follow up with the child care program will be conducted by a licensing consultant.
