Birth Certificates
Online
Cost is $10.00 per copy and a small fee for using a credit/debit card applies
Click on the link Birth/Death Certificate Online Applications. You will have to create a username and password to complete. Fill in the blanks. You may attach a copy of your ID (current driver’s license is our preference) to your online application or you may fax it to our office at 574-936-9247. Once we confirm the information and we receive your payment the certificate/s will be mailed to you unless you specify you will pick them up in the office.
By Mail
The cost is $10.00 per copy. Cash or Money Order Only!
Print out the Birth Certificate Application and mail it to the office along with funds in the form of Cash or Money Order Only! You will need to enclose your ID or we cannot process your request. Our preference is a photocopy of a current driver’s license. Certificates are normally mailed out the same day we receive your request through the mail.
Please Note: If we are unable to locate the record the cost is kept by the Health Department.
Death Certificates

The cost is $10.00 per copy and a small fee for using a credit/debit card applies.

Genealogical Services
- Genealogy Search for First 1-10 Names $10.00
- Genealogy Search for Every 5 Names Thereafter $5.00
- Certified Copies of Birth Certificates are $10.00 Each
- Notary Fee $2.00
Note: Search fee is kept by the Health Department regardless of whether the search is successful or not. If requesting a birth or death certificate you will need to fill out a request for each certificate ordered. If using a credit/debit card please note that the research fee and the certificate cost are kept by the Health Department if the record cannot be located.
Affidavit Requesting Amendment – $35.00
Paternity Affidavit and Paternity upon Marriage – $35.00
