- What do I need to receive a birth certificate for myself?
You must show a valid ID (driver's license, state ID, etc.)
- What are the operating hours?
Monday-Friday 8 AM-4 PM
- Directions to the Health Department?
The Health Department is located in Memorial Hall, located west (or right side) of the parking lot. The offices are in the basement. The address is 89 West Hill Street, Wabash, IN 46992.
- Who can obtain a birth certificate?
The person themselves (if over 18), Mother, Father, siblings, grandparents, spouse or guardian.
- How much is a certified birth certificate?
A certified birth certificate is $15.00 cash or money order ONLY. You will receive two (2) certified copies. One will be a smaller travel size.
- How much is a certified death certificate?
Certified death certificates are $12.00 each. Cash and money order are the ONLY forms of payment accepted.
