Immunizations

Why should your child be fully immunized?

Before vaccines became widely available, diseases like measles, mumps, and whooping cough were common in childhood, and thousands died or were left blind, deaf or brain-damaged by them. Today vaccines have totally or nearly eradicated several diseases such as smallpox, polio, diphtheria, and hib infections, according to the national Centers for Disease Control and Prevention.

Despite available vaccines, many diseases still persist putting unvaccinated babies and toddlers at risk. The CDC reported in the 2011 MMWR Weekly that 72% of the Indiana population ages 19-35 months have completed their childhood series of immunizations. That's GREAT progress, however, this means 28% of our young ones are still left vulnerable.

Kosciusko County Health Department

Offers immunizations to VFC-eligible Indiana residents. VFC (Vaccines For Children) is a federally funded program that provides recommended childhood vaccines for those age 18 or under, who have traditional or CHIP Medicaid and are uninsured.

There is a $15 fee per shot for those who have no insurance or insurance does not cover vaccines. Cash, checks and Credit/Debit Cards are accepted; Medicaid and insurances will be billed. No one will be turned away for inability to pay.

**The Immunization clinic can now accept some forms of insurance, please call your insurance prior to arrival to see if vaccines are covered**

Immunization Clinic Hours

Monday, Wednesday, and Friday: 8:30 am - 12:00 pm and 1:15 pm - 4:00 pm. Appointments and walk-ins.

Evening Clinic: 1st Monday of the month 4:30 pm - 6:30 pm (except on holidays).

It's very important that you bring your child's updated shot record to the clinic, also their Medicaid card.

  • The Immunization Clinic also provides most International Travel Vaccines for a fee.  For further information, contact the clinic at 574-267-7028.
  • TB skin tests needed for work or school are available on Mondays for a small fee.
  • Free lead testing for all children ages 1 year to 7 years. Please call 574-267-7028 to make an appointment.

Schedule for Childhood Immunizations

  • 2 months - DTaP/Hep B/IPV, HIB, PCV, Rotavirus
  • 4 months - DTaP/Hep B/IPV, HIB, PCV, Rotavirus
  • 6 months - DTaP/Hep B/IPV, HIB, PCV, Rotavirus
  • 12 to 18 months - MMR, Varicella, DTaP, PCV, Hep A (x2), HIB
  • 4 to 6 years - DTaP, IPV, MMR, Varicella
  • 6th Grade - MCV4, Tdap, HPV (2 doses if before 15, at least 6 months apart. >15, 3 doses)
  • Age 16 - MCV4 (#2) Men B (2 doses), HPV (3 doses if none prior to 15)
