Beware of Fraudulent Text and Email Messages
DWD WILL NOT send a text or email message asking you to click a link to receive benefits or any type of payment. Clicking on the link and providing personal information (for example your username, password, or social security number) will give fraudsters the ability to sign in to your accounts or sell your information on the dark web.
Be Aware of ID.me Fraudulent Messages
Please be aware of a new scam where fraudsters attempt to impersonate ID.me. ID.me ONLY operates through the ID.me website. Websites like “your-id-me.com” or other derivatives ARE NOT connected to ID.me. ID.me will not ask for personal information via messaging apps like WhatsApp, Telegram, or Facebook Messenger.

Report Unemployment Fraud
- Are you reporting that you were a victim of ID theft?
- Fill out State Form 57068 and mail or fax it to the department. Upon receipt of this form, DWD will lock the account from further use and initiate an investigation and issue a determination.
- If you are currently employed, contact your employer’s human resources department so that they can protest your claim with DWD.
- Other optional resources:
- File an electronic report with the Indiana State Police by filling out this form and emailing it to uifraud@isp.in.gov.
- (Note: Victims are strongly encouraged to completely fill out the template directly above to report UI fraud. Variations in reporting could cause delay in the report being filed.)
- File an identity theft complaint with the Indiana Attorney General’s Office at https://www.in.gov/attorneygeneral/2895.htm or by calling (800) 382-5516.
- The FBI also encourages victims to report fraudulent or any suspicious activities to the Internet Crime Complaint Center at http://www.ic3.gov. You may consult http://identitytheft.gov for help in reporting and recovering from identity theft.
- File an electronic report with the Indiana State Police by filling out this form and emailing it to uifraud@isp.in.gov.
- Protect your credit: Notify the Credit bureaus (Equifax, Experian, and TransUnion)
- Understand identity theft and taxes:
- Are you an employer reporting that one of your employees was a victim of ID theft or engaged in fraudulent activity?
- Protest the claim using State Form 640-P: Unemployment Insurance Protest (Employer)
- If ID theft, advise employee to follow the above steps for victims of ID Theft.
- If you are a victim of ID theft and would like to file a report to the Indiana State Police, please fill out this form and email it to uifraud@isp.IN.gov.

- Are you reporting that your Uplink account was accessed by a third party and your claim was either reopened and/or payments were diverted to a bank account that does not belong to you?
- Fill out State Form 57068 and mail or fax it to the department. Upon receipt of this form, DWD will initiate an investigation and issue a determination.
- If you are requesting payments be re-issued, you must also file a file an electronic report with the Indiana State by filling out the form available below and emailing it to uifraud@isp.in.gov.
- Are you reporting possible fraudulent activity by someone else?
You do not need to provide your name or any other identifying information in order to file a complaint in regards to unemployment insurance fraud. However, we will not be able to contact you if we need to seek additional information about your complaint if you choose to remain anonymous.
Please provide as much detailed information as possible concerning your allegations, including:
- The name and address of the individual or business suspected of committing fraud
- If the complaint is in regards to an individual, if available, include the individual’s SSN and the name of the business where the individual has been employed over the last year
- Describe your complaint in detail and provide dates if possible
