To notify the IDEM’s Office of Air Quality (OAQ) Permits Branch about a change to the Responsible Official (RO) or Authorized Individual (AI) at an existing permitted source when the source is not requesting any changes to the permit, complete and submit the following:
- Cover letter on company letterhead notifying the Office of Air Quality that the Responsible Official (RO) or Authorized Individual (AI) has changed. The new RO/AI must sign the letter.
- Air Permit Application Cover Sheet Form 50639 signed by the new RO/AI in Part D.
- GSD-01 Form 50640 including information for the new RO/AI in Part D. In Part D, Box 29, check the "YES" box to request a change to the RO/AI and enter the name of the new RO/AI.
The OAQ Permits Branch will not issue any changes to the permit, but will update our source information database accordingly.

Send one (1) signed hard copy of the original application packet by courier mail or hand delivery directly to IDEM’s Office of Air Quality (OAQ). Please do not send your air permit application to any other offices or individuals at IDEM, as this will delay receipt and processing of your application.
