The number of vaccination providers in Porter County continues to grow. We have broken down those providers into three categories. Please choose from the categories below.

Canceling Your Appointment
If you schedule an appointment with the Porter County Health Department, then find an earlier appointment somewhere else, PLEASE help us by canceling your appointment with us. By doing so, you will free up appointments that can be used by someone else! Just go to Submit an online vaccination cancellation form.
The Porter County Health Department (PCHD) is currently offering vaccinations at the following sites in Portage and Valparaiso. You will find specific registration instructions for each location.
PCHD Portage Location - Offers Pfizer & Moderna
North County Government Annex, 3590 Willowcreek Rd, Suite C, Portage. This location is open Mondays from 9:00 AM to 4:00 PM (closed for lunch from Noon to 1:00 PM). Walk-Ins will be accepted and appointments are still accepted. PCHD Valparaiso Location - Offers Pfizer, Moderna
Porter County Administration Center, 155 Indiana Ave, Suite 104, Valparaiso. This location is open Tuesday from 9:00 AM to 4:00 PM (closed for lunch from Noon to 1:00 PM). Walk-ins and appointments are still accepted. Due to limited supply, manufacturer specific vaccine may not be available on your scheduled date. Please call the department to verify which vaccine we currently have available. If you want to register for this specific site, go to the Registration Site and enter a zip code of 46383 when asked, then choose PORTER COUNTY HD MPVAX.

Registration By Phone
Eligible Hoosiers who do not have internet access or are otherwise unable to register online can call 2-1-1 for an appointment and/or secure transportation. A friend or family member also can also call 2-1-1 to make an appointment on behalf of a qualifying individual. But, the person registered will be required to provide photo identification before they will be given the vaccine.
Quick Tip
When you go to the vaccination registration site, you will be asked to enter your zip code. If you want to be vaccinated at a specific location, enter the zip code of that location. Your desired location should then appear near the top of the list of options. It will save you a lot of time scrolling down the various options before you find the one you want.
Vaccinations for 5 thru 17-Year-Olds
Only the Pfizer vaccine is approved for 5 and 17-year-olds. If you are setting up an appointment for a 5 to 17-year-old, please be sure to choose a vaccination site that offers the Pfizer vaccine.

Vaccinations are available at local pharmacies. Each pharmacy chain has its own registration and appointment website. Their websites can tell you which of their Porter County locations may be offering vaccinations and instructions to set up appointments. Some are also offering walk-in availability.
