Birth certificates can be obtained by filling out the following birth certificate forms (PDF) and submitting them to one of our offices.

Death certificates can be obtained by filling out the following death certificate forms (PDF) and submitting them to one of our offices.

Payment
In-Person: Cash, money order, or official checks. No personal checks are accepted. Photo identification from the person requesting the birth or death certificate is required. A copy of your current driver's license or State Photo ID with your signature will be accepted.
By Mail: Mail requests to the Valparaiso Office. Please provide your full name at birth, birth date, mother's full maiden name and father's full name, a photocopy of your current driver's license or State Photo ID, along with a self-addressed stamped envelope. Money order or official check accepted. No personal checks are accepted. DO NOT SEND CASH.
Due to the nature of the personal information, we recommend that you send your request via certified mail, Federal Express, or UPS. We are not responsible for lost or misdirected mail.
