Vital Records Division Mission: The Vital Records Division of the Porter County Health Department strives to provide friendly, courteous, and accurate service to all our clients while issuing birth and death certifications in full compliance with the Indiana State Department of Health.
Vital Records Assistance
Vital Records can be obtained at both office locations Monday thru Friday, 8:30 AM to 4:30 PM. (Please note: No Vital Records available at Portage location from 12:00 PM to 1:00 PM.) Please arrive by 4:15 PM so that we may provide you with the best customer service.
Request Forms
The person of record must have been born or died in Porter County to request these records.
*Only the following may obtain a copy of a birth certificate.(Self if over age 18, Parents, Grandparents, Siblings over the age of 18 or legal guardian with proof of guardianship.)*

The following services can only be obtained at the Valparaiso Office:
Paternity – Please call (219) 465-3525 for instructions on what documentation is needed and to schedule an appointment. We will discuss with you the changes that can be done at our office and changes that require a court order.
Amendments – Please call (219) 465-3525 for information.
Home Births – Please call (219) 465-3525 for instructions on what documentation is needed and to schedule an appointment. A certified nurse-midwife may also prepare a home birth record.
Genealogy – Birth and Death Certificates can be obtained from 1884 to the present.
To Obtain Apostille Birth Certificate:
Apostille is the authentication of a document by the Secretary of State. It cannot be processed at the local health department. It must be purchased from the Indiana State Department of Health. Birth and death certificates can also be purchased through the Indiana State Department of Health. Contact by phone at (317) 234-8156.
