Issue Resolution Trackers for PathWays, Health and Wellness and TBI Waivers

New tools have been created to allow FSSA staff to report issues when working with the Indiana PathWays for Aging program and the Health and Wellness and Traumatic Brain Injury Waivers. FSSA staff are encouraged to use the trackers to submit any issues encountered while working with the PathWays program or H&W and TBI Waivers and any issues a stakeholder reports. This issue resolution form is not intended for Q&A needs.

These forms are for internal use only and not intended for Q&A needs. Issues that may be reported include but are not limited to:

  • Claims
  • Compliance
  • Member Eligibility
  • Member Services
  • Stakeholder Escalations

You can report a PathWays issue using the PathWays Issue Resolution Tracker. Issues related to the Health and Wellness and Traumatic Brain Injury Waivers can be reported here. These tools help the team ensure consistent oversight and delivery of MLTSS services and support as we approach the July 1 launch and in the weeks that follow.
